What to expect For your convienience, you will be emailed an intake form prior to your appointment. Please complete the form at your convenience.

During your first appointment, you will be required to fill out a Health History Form, and a Consent Form. The initial assessments can take up to 5-10 minutes once you have completed your forms.

After this, I will go through your medical history, and Assessments and determine the objectives that will help us determine common goals and formulate a treatment Plan that will best meet those objectives.
